Abstract
Uvod: Bolnišnični transport življenjsko ogroženih pacientov je visoko tvegana intervencija zaradi nestabilne narave zdravstvenega stanja takšnih pacientov ter drugih zunanjih dejavnikov, kot sta delovanje zaposlenih in infrastruktura bolnišnice. Neželeni dogodki bolnišničnih transportov življenjsko ogroženih pacientov povzročajo nepotrebne zamude pri opravljanju drugih intervencij, ogrožajo zdravstveno stanje posameznika in višajo stroške zdravstvene oskrbe. Za zagotavljanje oz. izboljšanje varnosti življenjsko ogroženega pacienta med bolnišničnim transportom je ključna ocena zdravstvenega stanja pacienta pred pričetkom intervencije, uporaba preventivnih orodij in negovalnih intervencij ter izdelava transportnih protokolov. Namen: Namen diplomskega dela je predstaviti načine zagotavljanja varnosti med bolnišničnim transportom življenjsko ogroženega pacienta in najpogostejše neželene dogodke, ki se lahko med takšnimi transporti pojavijo. Metode dela: Uporabili smo deskriptivno metodo s pregledom literature. Iskanje literature smo izvedli v iskalnikih in bazah podatkov PubMed, Google scholar, ScienceDirect, CINAHL in Medline. Pri iskanju angleške literature smo uporabili ključne besede: transport, intrahospital, intra-hospital, critically ill, patient, checklist, guideline. Pri iskanju slovenske literature pa ključni besedi: znotrajbolnišnični transport. Pri izboru literature smo upoštevali čas objave med letoma 2010 in 2021, dostopnost celotnega besedila in vsebinsko ustreznost. Rezultati: Ugotovili smo, da je uporaba kontrolnega seznama pri izvajanju bolnišničnega transporta življenjsko ogroženega pacienta izrednega pomena za zmanjšanje pojavnosti neželenih dogodkov. Vpeljava tega preventivnega orodja ne zahteva velikih sistemskih sprememb ali visokih dodatnih stroškov bolnišnicam ter omogoča boljšo pripravo pacienta, opreme in zaposlenih na intervencijo. Večina neželenih dogodkov je povezana s tehničnimi težavami in z zdravstvenim stanjem posameznika. Drugi neželeni dogodki so težave z organizacijo intervencije, neustrezna infrastruktura bolnišnice in neusposobljenost transportne ekipe. Ugotovili smo, da zaposleni sčasoma transportne protokole sprejmejo v vsakdanjo prakso in da vzporedno s tem delež neželenih dogodkov intervencije upade. Razprava in zaključek: Za preprečitev neželenih dogodkov in zagotavljanje varnosti življenjsko ogroženega pacienta med izvajanjem bolnišničnega transporta je nujna ustrezna sestava transportne ekipe, pravilno delovanje opreme in hemodinamska stabilnost posameznika pred intervencijo. Uporaba kontrolnega seznama pri bolnišničnem transportu življenjsko ogroženega pacienta izboljša njegovo varnost in zmanjša pojavnost neželenih dogodkov.

Title:Efficiency of using a check list in hospital transport of critically ill patients : diploma work
Abstract:
Introduction: Hospital transport of critically ill patients is a high-risk intervention due to the unstable nature of the clinical status of such patients and other external factors. Adverse events of hospital transports of critically ill patients cause unnecessary delays, endanger the individual and increase the costs of medical care. Assessing the patient's health before the intervention, use of preventive tools and the development of transport protocols are key to improving safety of critically ill patients during hospital transports. Purpose: The purpose of this thesis is to present ways to ensure safety during hospital transport of a critically ill patient and the most common adverse events that may occur during such transport. Methods: We used a descriptive method to review scientific literature. The literature was obtained from search engines and databases (PubMed, Google scholar, ScienceDirect, CINAHL, Medline). The following keywords were used in the search: transport, intrahospital, intra-hospital, critically ill, patient, checklist, guideline. When selecting the literature published between 2010 and 2021 in Slovene or English, we considered the content adequacy and the accessibility of the full text. For a more comprehensive explanation of theoretical basis, the literature review also includes literature from the list of sources of selected articles. Results: The introduction of a check list reduces the incidence of adverse events without major systemic changes or additional costs for hospitals. Most adverse events are related to technical problems and the clinical status of the patient. Other specified adverse events are problems with organization of the intervention, inadequate hospital infrastructure and incompetence of the transport team. Over time, employees adopt transport protocols into their daily practice and in parallel, the occurrence of adverse events decreases. Discussion and conclusion: To prevent adverse events and ensure the safety of critically ill patients during intra-hospital transports, it is necessary to have an appropriate transport team, proper equipment, and a hemodynamically stable individual prior to the intervention. The use of transport checklists during intra-hospital transport of critically ill patients improves patient safety and reduces the incidence of adverse events.
