Abstract
Uvod: Ritmične zvočne spodbude se uporabljajo pri rehabilitaciji hoje bolnikov s Parkinsonovo boleznijo, saj služijo kot zunanje iztočnice za korake, ki nadomestijo okvarjene notranje mehanizme, ki skrbijo za ritmično gibanje. Namen: Povzeti izsledke raziskav o učinkovitosti uporabe zunanjih ritmičnih zvočnih spodbud za izboljšanje hoje pri bolnikih s Parkinsonovo boleznijo. Metode dela: Literaturo smo iskali v podatkovnih bazah PubMed, CINAHL in PEDro. Podatki so bili pridobljeni z analizo primarnih virov. Pregledani so bili tudi seznami literature vključenih raziskav in ostalih pregledanih člankov. Vključene so bile raziskave randomiziranih kontroliranih poskusov, napisane v angleškem jeziku. Rezultati: V pregled smo po upoštevanju vključitvenih in izključitvenih meril vključili 12 raziskav, v katerih je bilo skupno zajetih 580 preiskovancev. Vadba z ritmom se je v primerjavi z vadbo brez ritma pokazala za učinkovitejšo pri izboljšanju hitrosti hoje v dveh od štirih raziskav, v ostalih dveh je vadba z ritmom izboljšala rezultate pred-po intervenciji. Zvočne spodbude so prav tako vplivale na izboljšanje kadence v štirih raziskavah (dve raziskavi v primerjavi z vadbo brez ritma, dve v primerjavi z skupino brez vadbe), nekaterih prostorskih spremenljivkah koraka in funkcijskih testov hoje. Na časovno merjeni vstani in podi test ritem v primerjavi z vadbo brez njega ni imel dodatnega učinka. V primerjavi z intervencijo brez vadbe je bila vadba z ritmom v treh raziskavah učinkovitejša pri izboljšanju hoje v skoraj vseh parametrih. V primerjavi z vidnimi spodbudami je imel ritem večji učinek le na dolžino dvojnega koraka. Razprava in zaključek: Ritmične zvočne spodbude so učinkovitejše pri rehabilitaciji hoje bolnikov s Parkinsonovo boleznijo kot sama vadba ali program brez vadbe. Največje učinke bi lahko imela vadba z glasbo s poudarjenim ritmom, ki je primerno stopnjevana, strokovno nadzirana in zadostno intenzivna. Potrebne so obsežnejše in bolj kakovostne raziskave za določitev smernic za vadbo z ritmičnimi zvočnimi spodbudami pri bolnikih s Parkinsonovo boleznijo.

Title:Effects of rhythmic auditory cueing on gait improvement in Parkinson's disease - literature review : diploma work
Abstract:
Introduction: Rhythmic auditory cues are used in the rehabilitation of gait in Parkinson's disease patients, as they serve as external cues for steps that replace the damaged internal mechanisms that take care of rhythmic movement. Purpose: Summarize findings on the effectiveness of using external rhythmic auditory cues to improve gait in Parkinson's disease patients. Methods: Literature was searched in PubMed, CINAHL and PEDro databases. Data were obtained by analysis of primary sources. Reference lists of reviewed studies were also assessed. Randomized controlled trials written in English were included. Results: 12 studies involving 580 subjects were included in the review. Auditory cueing was shown to be more effective in improving walking speed when compared to non-rhythm exercise in two of four studies, furthermore, exercise with rhythm improved pre-post-intervention outcomes in the other two studies. Auditory stimuli also affected the improvement of cadence two studies compared to exercise without rhythm, two compared to intervention without exercise), some step spatial variables, and functional tests. Auditory cueing had no additional effect on Timed Up and Go test when compared to training without it. In comparison with intervention without exercise, rhythm exercise was more effective in improving gait in almost all parameters in three studies. In comparison to visual stimuli rhythm had more effect only on stride length. Discussion and conclusion: Auditory cueing is more effective in rehabilitating gait in Parkinson’s disease patients than exercise alone or a non-exercise program. Exercise with music that has an emphasized rhythm, which is appropriately progressed, professionally supervised and sufficiently intense, could have the greatest effects. More extensive and better-quality studies are needed to establish guidelines for exercise with rhythmic auditory cueing for Parkinson’s disease patients.
