Abstract
Uvod: Klinične smernice za fizioterapijo po možganski kapi priporočajo izvajanje vadbe v vodi za izboljševanje ravnotežja, premičnosti, mišične zmogljivosti in aerobne zmogljivosti. Doslej izvedeni sistematični pregledi so zajeli različne načine vadbe v vodi tudi s podvodnimi napravami. Namen: Namen diplomskega dela je bil pregledati izsledke raziskav o učinkovitosti vadbe v vodi brez uporabe podvodnih naprav za izboljšanje motoričnih sposobnosti pacientov po možganski kapi. Metode dela: Izvedli smo pregled literature randomiziranih kontroliranih poskusov v PubMed in PEDro. Rezultati: V pregled smo vključili devet randomiziranih kontroliranih poskusov, objavljenih med letoma 2004 in 2021. V vseh pregledanih raziskavah so ugotavljali učinkovitost različnih načinov vadbe v vodi za izboljšanje ravnotežja in premičnosti, v treh učinkovitost za izboljšanje mišične zmogljivosti, v dveh pa učinkovitost za izboljšanje aerobne zmogljivosti, v primerjavi s standardno obravnavo na suhem. Vadbo v vodi so izvajali samostojno ali dodano obravnavi na suhem. Oblike vadbe v vodi so bile različne: splošna vadba v vodi, vadba v vodi za izboljšanje ravnotežja in hoje z dvojnimi nalogami, vadba v vodi po pristopih Halliwick in/ali Ai Chi. Vadba v vodi za izboljšanje ravnotežja in premičnosti se je izkazala za učinkovitejšo od obravnave na suhem v sedmih izmed devetih raziskav, za izboljšanje mišične zmogljivosti v dveh izmed treh raziskav, za izboljšanje aerobne zmogljivosti pa v eni izmed dveh raziskav. Razprava in zaključek: V sedmih izmed devetih raziskav je bila dokazana večja učinkovitost vadbe v vodi za izboljšanje motoričnih sposobnosti, v primerjavi z vadbo na suhem. Potrebne so nadaljnje raziskave z večjim številom preiskovancev, preverjanjem dolgoročnih učinkov, neposredno primerjavo med različnimi načini vadbe v vodi, enotnejšimi merilnimi orodji in ločeno za subakutno in kronično fazo po možganski kapi.

Title:The effect of aquatic therapy to improve motor skills in patients post-stroke : diploma work
Abstract:
Introduction: Clinical guidelines for physical therapy in patients with stroke recommend performing aquatic therapy to improve balance, mobility, muscle strength and aerobic endurance. Previous systematic reviews have included various methods of aquatic therapy with aquatic exercise equipment. Purpose: The purpose of the diploma work was to review randomized controlled trials in order to establish the effectiveness of aquatic therapy without the use of aquatic exercise equipment to improve motor skills in patients post-stroke. Methods: A literature review of randomized controlled trials was conducted in PubMed and PEDro. Results: Nine randomized controlled trials published between 2004 and 2021 were included in the review. All review studies were assessing the effect of different modes of aquatic therapy to improve balance and mobility, three studies the effect to improve muscle strength and two studies the effect to improve aerobic endurance, compared to standard treatment on dry land. Aquatic therapy was performed alone or in addition to the dry land treatment. The methods of aquatic therapy were different: general aquatic therapy, dual task training for balance and mobility, Halliwick and/or Ai Chi therapy. Aquatic therapy proved to be more effective than dry land therapy to improve balance and mobility in seven of the nine studies, to improve muscle strength in two of the three studies, and to improve aerobic endurance in one of the two studies. Discussion: In seven of the nine studies was demonstrated a significant effectiveness of aquatic therapy to improve motor skills in patients post-stroke, compared to the dry land therapy. Further research with larger sample sizes, evaluation of long-term effects, direct comparison between different types of aquatic therapy, more uniform assessment tools and separately for the subacute and chronic phase after stroke are needed.
