Vpliv protetične oskrbe z akrilatno delno protezo in delno protezo z ulito bazo na kazalce ustnega zdravja pri prebivalcih Kosova

Abstract
Ozadje V stomatološki protetiki lahko brezzobost zdravimo s snemnimi in fiksnimi protetičnimi nadomestki. Tako v maksilarni kot mandibularni čeljusti se pri zdravljenju brezzobosti najpogosteje uporablja snemne proteze, ki sedijo na sluznici in zobeh. Cilj te doktorske disertacije, ki je razdeljena na retrospektivno epidemiološko ter prospektivno klinično študijo, je analizirati trende protetične oskrbe na Kosovu in oceniti vpliv snemnih delnih protez na kazalnike ustnega zdravja pri prebivalcih Kosova. Udeleženci in metode V okviru epidemiološke raziskave smo iz arhivov primarnih zdravstvenih ustanov v treh kosovskih regijah (Prizren, Peje, Ferizaj) pridobili podatke o zdravljenju z delnimi akrilnimi protezami in totalnimi protezami v obdobju 2002–2013. Podatke smo analizirali glede na leto oskrbe, vrsto proteze, prizadeto čeljust, starost pacientov, njihov spol ter mesto bivanja na podeželju ali v urbanem okolju. Prospektivno klinično študijo smo izvedli na 40 pacientih, od katerih jih je 20 prejelo delne akrilne proteze (9 na maksilarni in 11 na mandibularni čeljusti), 20 pa delne proteze z ulito bazo (9 na maksilarni in 11 na mandibularni čeljusti). Starost pacientov je znašala 45–65 let, sodelovalo je 24 žensk in 16 moških. Ustno higieno smo nadzirali s pomočjo analize različnih parametrov: bioloških zapletov, kot so karies, zlom zoba in izguba zob, kliničnih in biokemičnih indikatorjev parodontalnih težav z dodatnimi biokemičnimi meritvami C–reaktivnega proteina v serumu (Hs-CRP) in alkalne fosfataze (ALP) ter tridimenzionalnih sprememb v kostnem in mehkih tkivih pod bazo na skrajšani zobni vrsti. Odkrili smo mehanske težave na zobnih in gingivalnih komponentah protez. Za analizo vpliva, ki ga imajo delne proteze na kakovost življenja pacientov, smo uporabili vprašalnik profila ustnega zdravja OHIP-14. Rezultati Rezultati epidemiološke raziskave so pokazali povečano absolutno število totalnih protez glede na relativno število delnih akrilnih protez. Povprečno številno delnih akrilnih protez je vseeno raslo hitreje kot povprečno število totalnih protez, zato je bila možnost, da se pri zdravljenju mlajših pacientov uporabijo delne akrilne proteze, precej višja. Rezultati prospektivne raziskave so naslednji: 1) našli nismo nobenih izrazitih razlik med protezami z ulito bazo in akrilnimi delnimi protezami kar zadeva kariozne lezije; 2) pacienti s protezami z ulito bazo so imeli višje rezultate merjenja plaka na nosilnih zobeh kot pacienti z akrilnimi delnimi protezami, medtem ko so imeli pacienti z akrilnimi delnimi protezami pomembno višje vrednosti BOP kot pacienti s protezami z ulito bazo; 3) pri mobilnosti nosilnih zob nismo našli nobenih pomembnih razlik med protezami z ulito bazo in akrilnimi delnimi protezami; 4) pri biokemičnih preiskavah CRP in ALP nismo našli nobenih pomembnih razlik med protezami z ulito bazo in akrilnimi delnimi protezami; 5) med protezami z ulito bazo in akrilnimi delnimi protezami nismo našli nobenih pomembnih razlik glede sprememb kostnega tkiva in mehkih tkiv v mandibularni čeljusti, kar smo ugotavljali na osnovi kvalitativnega ocenjevanja ter kvantitativnih sprememb v volumnu kosti ali sluznice; 6) pri obeh skupinah protez so bile vrednosti OHIP-ALB14 višje pri T1, nato pa so padle pri T2 in T3. Zaključek V obdobju 2002–2013 se je trend frekvence novih totalnih protez in akrilnih delnih protez v primarnih zdravstvenih ustanovah na Kosovu linearno povečal. Absolutno število primerov, ko je bilo potrebno zdravljenje s totalnimi protezami, je bilo dvakrat večje kot število primerov, ko so bile potrebne akrilne delne proteze. Kljub temu se je v zadnjih letih delež akrilnih delnih protez pomembno povečal glede na število totalnih protez. Ugotovitve kažejo, da po dvanajstih mesecih zdravljenja s snemnimi delnimi protezami ni razlik med akrilnimi delnimi protezami in delnimi protezami z ulito bazo v smislu mehanskih in bioloških zapletov ter samoocene pacientov glede ustnega zdravja.

Title:The influence of prosthodontic treatment with removable partial denture with and without metal framework on oral health indicators of the residents of Kosovo
Abstract:
Background The prosthodontics considerations for the treatment of edentulism can be divided as removable and fixed appliances. The conventional mucosa and tooth-supported dentures are the most widely used form of removable appliances for both maxillary and mandibular edentulous regions. The aim of this doctoral study, which was split as retrospective epidemiological and prospective clinical parts, was to analyse trends of prosthodontic treatment on Kosovo and to assess the influence of removal partial dentures on oral health indicators among residents in the Kosovo. Participants and Methods For the epidemiological study, the data on ARPDs delivery and CDs delivery were obtained from the archives of primary health centres of three Kosovo regions (Prizren, Peje, Ferizaj) from 2002 to 2013. The data were analysed concerning year of treatment, type of dentures, jaw, age, gender and urban or rural origin of the patients. The prospective clinical study was performed on 40 patients of which 20 received ARPDs (9 maxilla and 11 mandible) and 20 received MRPDs (9 maxilla and 11 mandible). The patients were aged between 45-65 years with 24 females and 16 males. To monitor oral health, the various parameters were analysed: biological complications like caries, tooth fractures, tooth loss, clinical and biochemical indicators of periodontal complications, with additional biochemical measurement of serum C-reactive protein (Hs-CRP), and alkaline phosphatase (ALP) and three-dimensional changes in the bone and soft tissues under denture free-end saddles. Technical complications of denture dental and gingival components were detected. The influence of RPDs on the patients’ quality of life were analysed with Oral Health Impact Profile (OHIP-14) questionnaire. Results The significant results of the epidemiological study showed that there was an increase in the absolute numbers of CDs when taken into consideration the relative numbers of ARPDs. However, the average number of ARPD’s increases more rapidly compared with the average number of CD’s and the potential for this denture type to be used in younger patients was significantly higher. In the prospective study: i) no distinct differences between MRPDs and ARPDs in new carious lesions; ii) abutment teeth with higher plaque scores in MRPD’s wearers when compared to ARPD’s wearers, whereas ARPD users had significantly higher mean BOP values than MRPD users; iii) no significant differences between MRPDs and ARPDs in mobility of abutment teeth; iv) no significant differences between MRPDs and ARPDs regarding biochemical measures CRP and ALP; v) no significant differences between MRPDs and ARPDs regarding bone and soft tissue changes in mandibular jaws based in qualitative scoring classification as well as based on quantitative bone and mucosa volume changes; vi) at T1 interval both compared denture groups had higher OHIP-ALB14 scores and the average scores significantly decreased in T2 and T3, but with no significant differences between MRPD and ARPD. Conclusion The trend in the frequency of new CD and ARPD treatment increased linearly from 2002 to 2013 in the Primary Health Centres in three regions of Kosovo. The need for CD treatment in absolute numbers is more than twice as high as the need for treatment with ARPD. However, the proportion of ARPDs compared to CDs significantly increased over the years. Conclusions can be summarized that after 12 months of treatment with removable partial dentures, there are no differences between ARPD and MRPD in terms of the occurrence of technical and biological complications and patients oral health self-assessment.
