Vpliv parenteralne prehrane na domu na spremembo faznega kota in funkcionalne mase pri rakavih bolnikih z odpovedjo prebavil

Abstract
Pri rakavih bolnikih, ki imajo odpoved prebavil zaradi napredovanja bolezni ali posledic zdravljenja, je parenteralna prehrana terapija odpovedi prebavil. V naši raziskavi smo analizirali vrednosti faznega kota (FK) in indeksa puste telesne mase (ang. Fat Free Mass Index – FFMI) pri paliativnih rakavih bolnikov, ki so prejemali parenteralno prehrano na domu (ang. Home Parenteral Nutrition – HPN). Primerjali smo vrednosti pridobljene z bioimpedančno metodo meritve sestave telesa ob začetni oceni prehranskega stanja s stanjem po začetku uvajanja HPN v prvih treh mesecih. Za oceno vpliva HPN pri rakavih bolnikih na spremembo funkcionalnega stanja smo uporabili vrednosti FK, za oceno spremembe funkcionalne mase smo uporabili vrednosti FFMI. V raziskavi smo ugotovili, da HPN prispeva k izboljšanju ocene funkcionalne mase, FFMI se najbolj zvišal pri tistih bolnikih, ki so prejemali največji volumen HPN (䁥 2500 ml). Nakazan je tudi trend izboljšanja FK, največje izboljšanje je opaženo pri bolnikih, ki so prejemali najmanjši volumen HPN (䁥 500 ml in < 1000 ml). Izsledki raziskave potrjujejo, da je prehranska podpora paliativnega rakavega bolnika z odpovedjo prebavil pomembna, predvsem ker omogoča ohranjanje funkcionalne mase in vpliva tudi na izboljšanje funkcionalnega stanja. Na ta način se izboljša presnovna rezerva rakavega bolnika, boljše obvladovanje rakave bolezni, ohranjata se bolnikova zmogljivost ter kvaliteta življenja.

Title:The impact of home parenteral nutrition on phase angle change and functional mass in cancer patients with gastrointestinal failure
Abstract:
In cancer patients who have gastrointestinal failure due to disease progression or treatment consequences, parenteral nutrition is therapy for gastrointestinal failure. In our study, we analysed values of phase angle (slo. Fazni Kot – FK) and fat free mass index (FFMI) of palliative cancer patients on home parenteral nutrition (HPN). We compared values obtained by the method of bioelectrical impedance analysis measurements at the initial evaluation of nutritional status with their condition in 3 months after the introduction of HPN. For the analysis, FK values were used to assess the effect of HPN in cancer patients on changes in functional status, and FFMI values were used to assess changes in functional mass. In the study, we found that HPN can contribute to the improvement of functional mass, the highest increase in FFMI was in those patients who received the highest volume of HPN (䁥 2500 ml). There is also a trend of improvement in FK, the greatest improvement was observed in patients receiving the lowest volume of HPN (䁥 500 ml and < 1000 ml). The results of the research confirm that the nutritional support of a palliative cancer patient with gastrointestinal failure is important, especially because it enables the maintenance of functional mass and improves the functional status of patients on HPN. This contributes to the metabolic reserve of the cancer patient, improves cancer control, maintains patient capacity and quality of life.
