Abstract
Uvod: Artrogena mišična inhibicija je pojav mišične oslabelosti zaradi zmanjšane sposobnosti hotene aktivacije mišice, ki se navadno pojavi po okvarah kolena, kljub odsotnosti poškodb na mišici ali živcu. Artrogena mišična inhibicija je varnostni mehanizem telesa, ki preprečuje morebitne nevarne obremenitve sklepa takoj po poškodbi, kasneje pa lahko ovira normalno okrevanje, saj lahko vodi v dolgotrajno atrofijo in šibkost. Vadba z lahkim bremenom in z zmanjšanim krvnim pretokom skozi aktivne mišice (ishemična vadba) se uveljavlja kot alternativa standardni vadbi s težkimi bremeni, in sicer predvsem v primerih, ko je delovanje večjih sil na mišično-skeletni sistem kontraindicirano. Namen: Namen poročila dveh primerov obravnave pacientov je primerjati učinke ishemične vadbe z lahkimi bremeni na navor maksimalne hotene izometrične kontrakcije in izometrično vzdržljivost mišice quadriceps femoris z enako vadbo brez oviranega pretoka krvi. Metode: Štiritedenski vadbeni program z ishemično vadbo z lahkimi bremeni in vadbo z lahkimi bremeni brez ishemije je potekal na dveh pacientih z okvaro kolenskega sklepa (strgana sprednja križna vez). Vadbeni program je trajal 4 tedne, vsak teden sta opravila tri vadbene enote, skupaj 12 vadbenih enot. Med vsako vadbeno enoto smo ocenili zaznavanje občutka napora, mišičnega neugodja in bolečine v kolenskem sklepu na 10-stopenjski Borgovi lestvici ter beležili opravljeno delo. Pred vadbenim programom in po njemu smo z izometričnim dinamometrom za koleno izmerili navor maksimalne hotene izometrične kontrakcije in izometrično vzdržljivost mišice quadriceps femoris ter z Lysholmovim vprašalnikom dobili subjektivno oceno stanja kolenskega sklepa. Rezultati: Ishemična vadba se je izkazala za učinkovitejšo od vadbe z lahkimi bremeni brez ishemije pri doseganju subjektivnega funkcijskega napredka kolena. Vadba z lahkimi bremeni brez ishemije je bila učinkovitejša pri pridobivanju izometrične moči in vzdržljivosti. Razprava in zaključek: Raziskava je pokazala večjo učinkovitost ishemične vadbe pri izboljševanju subjektivne funkcije sklepa, vadba z lahkimi bremeni pa je bila učinkovitejša pri pridobivanju moči in vzdržljivosti. Zaradi različno izražene artrogene mišične inhibicije med preiskovancema rezultatov ne moremo posplošiti.

Title:The effects of low-load ischemic exercise on chronical muscular atrophy of quadriceps femoris muscle in patients - double case study : diploma work
Abstract:
Introduction: Arthrogenic muscle inhibition is the onset of muscle weakness due to impaired voluntary muscle activation that usually onsets after knee impairments, even though there is no injury on muscle or nerve. Arthrogenic muscle inhibition is a defensive mechanism which prevents potentially dangerous loads of the joint immediately after injury. Later in rehabilitation it slows down normal regeneration because it prevents strenghtening of the muscle and eventually leads to long-lasting atrophy and weakness. Ischemic exercise with low loads is becoming an alternative to standard exercise protocols with high loads for improvement of muscle strength and hipertrophy, especially when high loading of the joint is contraindicated. Purpose: The purpose of this double case report was to compare the effects of ischemic exercise with low loads and exercise with low loads on maximal voluntary isometric contraction, isometric endurance and function of the knee joint. Methods: A four week intervention with ischemic and standard low load exercise took place in two subjects with knee joint injury. The protocol lasted four weeks with three training units per week. During every training unit we asked the subject about their perception of effort, muscle pain and knee pain on 10 point Borg scale. We measured maximal voluntary isometric contraction and isometric endurance with isometric dinamometer pre- and post-intervention, subjects also took subjective Lysholm test pre- and post-intervention. Results: Ischemic exercise was more effective in subjective functional progress. Low load exercise was more effective in maximal voluntary isometric contraction and isometric endurance. Discussion and conclusion: Blood flow restricted exercise was more effective in improving subjective function, low load exercise was more effective in improving strength and endurance. Due to different begining levels of arthrogenic muscular inhibition of the subjects, we cannot generalize these results.
