Abstract
Uvod: Lateralni epikondilitis je tendinopatija skupne kite ekstenzorjev podlahti, za katero je značilna bolečina v zunanjem delu komolca, ki se običajno poslabša ob radialni ekstenziji zapestja. Glavni vzrok za pojav lateralnega epikondilitisa so prekomerne in ponavljajoče se obremenitve mišic ekstenzorjev zapestja. Zdravljenje lateralnega epikondilitisa je večinoma simptomatsko, torej usmerjeno v zmanjšanje bolečine ter povečanje moči in fleksibilnosti mišic. Namen: Namen diplomskega dela je ugotoviti učinek terapije z globinskimi udarnimi valovi na bolečino pri zdravljenju lateralnega epikondilitisa. Metode dela: Pregled strokovne literature. Literatura je bila zbrana iz zbirk podatkov Cinahl, PubMed, Medline in ScienceDirect. Rezultati: Akutni učinek zdravljenja bolečine z globinskimi udarnimi valovi je bil v primerjavi s kontrolno skupino boljši za povprečno 13 %, kronični pa za 16 %. Uporabljena je bila tako fokusna kot radilana oblika valov. Pri fokusni obliki valov je bilo izboljšanje povprečno 21% v akutni fazi po koncu zdravljenja, v kronični pa 37%. Pri radilani obliki je bilo izboljšanje v akutni fazi po zaključku zdravljenja povprečno boljše za 18 %, v kronični pa za 31 %. Razprava in zaključek: Pri uporabi terapije z globinskimi udarnimi valovi za zdravljenje bolečine pri lateralnem epikondilitisu sicer pride do izboljšanja, vendar izboljšanje ni bistveno boljše kot pri kontrolni skupini. Presenetljivo je, da je bila velikokrat uporabljena tudi fokusna oblika valov. Rezultati nakazujejo na nekoliko boljši učinek fokusne oblike valov. Za potrditev pozitivnega učinka terapije z globinskimi udarnimi valovi na bolečino bi bilo potrebnih več raziskav s kontrolnimi skupinami in natančnejšo zasnovo.

Title:The effect of extracorporeal shock-wave therapy on pain at lateral epikondylitis : diploma work
Abstract:
Introduction: Lateral epicondylitis in tendinopathy of common forearm extensor muscles tendon. It is typically recognized by pain on lateral epicondyle of elbow, which is worse, when wrist is in dorsal flexion. The main reason for lateral epicondylitis is overuse of extensor muscles, and it is found when load is repetitive. Treatment of lateral epicondylitis is mostly symptomatic. It is focused on reducing pain and improving strenght and flexibility of muscles. Purpose: Purpose of diploma work is to find out the effect of extracorporeal shock-wave therapy on the pain at lateral epicondylitis. Methods: Diploma work is review of nonfiction. Databases are Cinahl, PubMed, Medline and ScienceDirect. Results: Acute effect on pain was for 13 % better in case of extracorporeal shock-wave therapy, than in a control group, while chronic was 16%. Focused and radial waves were used. When focused were used, acute imrovement of pain was 21%, while chronic was 37%. I case of radial waves acute effect was 18%, while chronic was 31%. Discussion and conclusion: Results show improvement in pain by using the extracorporeal shock-wave therapy, but in comparison with control group is it not crucial. Frequency of use of radial and focus waves is surprisingly almost the same. In the future more of researches with control group and more strictly conditions will be needed.
