Primerjava učinkov zgodnje mobilizacije z imobilizacijo po pretrganju Ahilove tetive - pregled literature

Abstract
Uvod: V zgodnji fazi po pretrganju Ahilove tetive je lahko poškodovani del popolnoma imobiliziran ali pa se izvaja fizioterapija z zgodnjo mobilizacijo tetive in sklepa v omejenem obsegu giba. Namen pregleda literature je bil primerjati učinke imobilizacije z učinki zgodnje mobilizacije po popolnem pretrganju Ahilove tetive. Metode: Literatura je bila iskana v podatkovnih zbirkah Medline, Science Direct, CINAHL, Scopus, Proquest, Cochrane Library in Wiley Online Library. Upoštevane so bile le randomizirane kontrolirane raziskave, objavljene med letoma 2005 in 2015, ki so primerjale oba protokola zgodnje obravnave. Rezultati: V pregled je bilo vključenih osem raziskav učinkovitosti zgodnje obravnave po operativno zdravljeni pretrganini Ahilove tetive in ena raziskava učinkovitosti pri konzervativno zdravljeni pretrganini tetive. Kratkoročni učinki fizioterapije z zgodnjo mobilizacijo po operativnem zdravljenju so hitrejše pridobivanje mišične zmogljivosti, manjše omejitve pri izvajanju športnih dejavnosti, hitrejša vključitev v delo in boljša subjektivna ocena zdravljenja. Dolgoročni učinki obeh protokolov pa se verjetno bistveno ne razlikujejo. Zaključki: Iz objavljene literature ni mogoče ugotoviti, ali je hitrejše pooperativno okrevanje posledica predvsem zgodnje mobilizacije kite in sklepa ali pa je pomembnejša zgodnja obremenitev spodnjega uda. V prihodnje bi bilo smiselno preučiti optimalne parametre stopnjevanja zgodnje mobilizacije s hkratnim obremenjevanjem poškodovanega dela ali brez obremenjevanja.

Title:Comparison of effects of immobilization and early mobilization after Achilles tendon rupture - literature review
Abstract:
Background: In the early period after ruptured Achilles tendon the ankle can be either fully immobilized or subject to physiotherapy with joint and soft-tissue mobilization. The present literature review aimed to compare published data on efficiency of immobilization with those of early mobilization after a complete rupture of the Achilles tendon. Methods: The literature search was performed within the following databases: Medline, Science Direct, CINAHL, Scopus, Proquest, Cochrane Library and Wiley Online Library. Only randomized controlled studies that were published from 2005 to 2015 were considered relevant. Results: Of nine studies analysed, eight investigated efficiency of both protocols in the early period after surgical tendon suture and one in the same period during purely conservative treatment. Short-term effects of early mobilization after a surgical procedure comprise enhanced muscle strength gains, less restrictions in sport participation, faster return to work and better subjective ratings of the outcome. However, the long-term results of treatment by the two protocols are most likely very similar. Conclusions: It cannot be elucidated from published evidence whether the enhanced short-term effects are brought about by the early mobilization itself, or the weight-bearing of the joint plays more important role. Future studies should determine optimal parameters of mobilisation progression with or without concomitant weight-bearing.
