Učinkovitost balneoterapije pri pacientih s kronično bolečino v križu : sistematični pregled literature

Abstract
Uvod: Za zdravljenje bolečine v križu se uporablja tudi balneoterapija. Kopeli naj bi prek termalnih, mehaničnih in kemičnih učinkov vplivale na zmanjševanje bolečine ter izboljšanje gibljivosti, funkcijske premičnosti in splošnega počutja. Namen: Pregledati izsledke raziskav o učinkovitosti balneoterapije z uporabo kopeli za zdravljenje pacientov s kronično bolečino v križu. Metode: Na podlagi postavljenih meril so bile pregledane podatkovne zbirke PubMed, CINALH, PEDro in Cochrane. Rezultati: Vključenih je bilo šest randomiziranih kontroliranih raziskav z ocenami po lestvici PEDro med 4 in 7. Objavljene so bile med letoma 2005 in 2014 ter skupno zajele 386 pacientov. Proučevali so učinkovitost balneoterapije kot samostojnega (dve raziskavi) ali dopolnilnega (štiri raziskave) terapevtskega postopka. Izsledki kažejo, da ima balneoterapija statistično značilno večje pozitivne učinke kot obravnave v primerjalnih skupinah. Učinkovita je bila pri zmanjšanju bolečine in izboljšanju gibljivosti hrbtenice, funkcioniranja ter kakovosti življenja. Nekateri učinki so bili prisotni tudi tri do 15 tednov po obravnavi (tri raziskave). Zaključek: Balneoterapija z uporabo kopeli bi lahko bila učinkovit samostojen ali dopolnilen terapevtski postopek za zdravljenje pacientov s kronično bolečino v križu, vendar so zaradi večinoma nizke kakovosti dosedanjih raziskav za sklepanje trdnih zaključkov potrebne dodatne, bolje zasnovane raziskave.

Title:Effectiveness of balneotherapy in patients with chronic low back pain
Abstract:
Introduction: Low back pain could be treated also with balneotherapy. Through their thermal, mechanical and chemical effects, baths are supposed to exert influence to pain reduction, range of motion improvement, improvement of functional mobility and general well-being. Purpose: To determine whether balneotherapy with use of baths is effective in treatment of patients with chronic low back pain. Methods: Based on inclusion criteria, a literature review was conducted in the PubMed, CINALH, PEDro and Cochrane databases. Results: Six randomized controlled trials with PEDro scores between 4 and 7 were included. They were published between 2005 and 2014 and had a total of 386 patients. Effectiveness of balneotherapy as a single (2 studies) or complementary intervention (4 studies) was investigated. Data show that balneotherapy has statistically significant positive effects, greater than control treatments. It was effective on pain reduction, increased spinal range of motion, better functioning and quality of life. Some effects were present also at 3 to 15 weeks follow up (3 studies). Conclusion: Balneotherapy with use of baths might be an effective single or complementary intervention for treatment of patients with chronic low back pain. For firm conclusions, however, additional studies with better design are required.
