Abstract
Uvod: Kronična bolečina v križu je ena najpogostejših zdravstvenih težav v razvitem svetu. Za njeno zdravljenje se najpogosteje uporablja telesna vadba. Stopnjevana dejavnost je oblika telesne dejavnosti, pri kateri se izvaja stopnjevan program vadbe z vključenim vedenjskim delom. Namen pregleda literature je bil preučiti učinkovitost protokola stopnjevane dejavnosti pri zdravljenju kronične nespecifične bolečine v križu. Metode: V pregled literature smo vključili raziskave, objavljene med januarjem 2004 in septembrom 2015, v slovenskem, angleškem ter italijanskem jeziku, najdene v spletnih podatkovnih zbirkah PEDro, PubMed, CINAHL in Cochrane. Rezultati: V analizo je bilo vključenih sedem raziskav, ki so stopnjevano dejavnost primerjale z drugo obliko vadbe. Po vseh protokolih vadb se je pacientom zmanjšala bolečina, izboljšala zmožnost gibanja, skrajšal čas bolniškega dopusta in zmanjšala stopnja kineziofobije. Zaključki: Rezultati so pokazali, da stopnjevana dejavnost ni učinkovitejša od drugih vrst terapije pri zmanjševanju bolečine in izboljšanju nezmožnosti, je pa učinkovitejša pri zmanjševanju kineziofobije in skrajšanju bolniškega dopusta. Smiselno bi bilo, da bi fizioterapevt pri izbiri programa terapije upošteval tudi stopnjevano dejavnost kot obliko terapije.

Title:The effect of graded activity protocol for chronic nonspecific low back pain
Abstract:
Background: Chronic low back pain is one of the most common health problems in the developed world. Physical activity is the most common treatment method. Graded activity is a form of physical activity that consists of a graded exercise programme and behavioural principles. The purpose of the study was to systematically review articles that studied the effectiveness of graded activity protocol in chronic low back pain treatment. Methods: An electronic databases search (PEDro, PubMed, CINAHL and Cochrane) in Slovenian, Italian and English, published between 2004 and 2015 was conducted. Results: Seven articles were included in the analysis. Patients in all treatment groups reported a reduction of pain and disability, lower levels of kinesiophobia and fewer days of absence from work. Conclusions: Results showed that graded activity protocol is not more effective than other forms of treatment in reduction of pain and disability, but more effective in the reduction of kinesiophobia. Patients that followed a graded activity protocol also reported fewer days off work. Graded activity protocol should be considered in chronic low back pain treatment protocols.
