Primerjava učinkovitosti vadbenih protokolov za krepitev mišic po artroskopski rekonstrukciji sprednje križne vezi

Abstract
Uvod: Po poškodbi sprednje križne vezi večini pacientov izvedejo kirurško rekonstrukcijo, ki ji sledi pooperativna fizioterapevtska obravnava s terapevtsko vadbo. Obstaja veliko različnih vadbenih protokolov, vendar njihova učinkovitosti še ni povsem jasna. Namen: Na podlagi pregleda objavljene strokovne in znanstvene literature primerjati učinkovitost vadbenih protokolov za krepitev mišic po artroskopski rekonstrukciji sprednje križne vezi. Metode: Literaturo smo iskali v podatkovnih bazah PEDro, PubMed in Cochrane Library. Omejili smo se na randomizirane kontrolirane raziskave, objavljene od leta 2000 do 2016. Rezultati: V pregled je bilo vključenih deset raziskav. V eni so primerjali standardni pooperativni protokol s pospešenim protokolom, v dveh so primerjali protokola, ki sta se razlikovala v času vključevanja vadbe v odprti kinetični verigi, v treh so primerjali različne vrste vadbe, v štirih raziskavah pa učinek dodajanja specifične vrste vadbe standardnemu pooperativnemu protokolu. Zaključki: Priporoča se izvajanje krajšega in intenzivnejšega vadbenega protokola, ki naj vključuje vadbo v odprti in zaprti kinetični verigi, progresivno ekscentrično vadbo ter kombinacijo vadbe za krepitev mišic in okretnosti ter pliometrično, proprioceptivno in športno-specifično vadbo. Zaključek vadbenega protokola naj temelji na doseganju funkcionalnih meril.

Title:Comparison of efficacy of training protocols for muscle strengthening after arthroscopic reconstruction of anterior cruciate ligament
Abstract:
Introduction: After the anterior cruciate ligament (ACL) tear, most of the patients undergo surgical ACL reconstruction, which is followed by postoperative physiotherapeutic exercise programs. The efficacy of various exercise programs is not entirely clear. Objectives: To analyse available evidence of the effectiveness of training protocols after arthroscopic anterior cruciate ligament reconstruction based on the review of available scientific literature. Methods: The literature was searched within PEDro, PubMed and Cochrane Library databases. Only reports of randomized controlled studies published from 2000 to 2016 were considered relevant. Results: Ten studies were included in the final review. One study compared an accelerated to a non-accelerated physiotherapy program, two studies compared early and late introduction of open kinetic chain exercises to the program, three studies compared programs of different types of exercise and four studies evaluated effects of adding specific exercise types to a standard postoperative program. Conclusion: Use of accelerated postoperative physiotherapy program which includes open and closed kinetic chain exercises, progressive eccentric exercise and combination of strengthening and neuromuscular exercises is advised. Decision on return to sport should be based on patient's ability to meet well defined functional criteria.
