Vpliv togih lepilnih trakov na obremenitev krožnih vezi prstov pri plezalcih - sistematični pregled literature

Abstract
Uvod: Pri plezanju so za držanje majhnih oprimkov potrebne velike sile fleksornih mišic prstov, ki se prek kit prenašajo na krožne vezi prstov. Te so med plezanjem zelo obremenjene in zato pogosto poškodovane. Da bi jih razbremenili in zaščitili pred poškodbo, plezalci pogosto uporabljajo toge lepilne trakove. Namen prispevka je na podlagi pregleda literature predstaviti vpliv nameščanja togih lepilnih trakov na obremenitev krožnih vezi prstov ter njihovo učinkovitost v preventivi poškodb in zaščiti po poškodbi krožnih vezi prstov na roki. Metode: Pregledali smo podatkovne zbirke MEDLINE, CINAHL in Science Direct. Rezultati: Vključene so bile štiri raziskave, v katerih so ugotavljali vpliv različnih tehnik nameščanja togih lepilnih trakov na obremenitev krožnih vezi prstov. Krožno nameščen togi lepilni trak minimalno razbremeni krožne vezi prstov, vendar jim ne daje zadostne opore, da bi lahko preprečil poškodbo. Tehnika nameščanja v obliki črke H zmanjša odmik kite in s tem trenje med kito in vezmi ter tako pomaga preprečiti razvoj vnetja in bolečine. Zaključki: Odsvetujemo nameščanje togih lepilnih trakov v preventivne namene. V pozni fazi rehabilitacije po poškodbi krožnih vezi priporočamo nameščanje togega lepilnega traku v obliki črke H. Za izboljšanje kakovosti dokazov bi bilo treba izvesti randomizirane kontrolirane poskuse.

Title:The influence of nonelastic adhesive taping on loading of finger tendon pulleys in rock climbers - systematic literature review
Abstract:
Introduction: High forces are generated in the finger flexors while climbing on small holds. They apply stress to the finger pulleys, which are often injured. Climbers often use taping to protect their pulleys from injuries. The purpose of this systematic review was to determine the impact of taping on pulley load and the effect of this method for injury prevention and rehabilitation. Methods: MEDLINE, CINAHL and Science Direct databases were searched. Results: Four studies evaluating the impact of different taping techniques on pulley load were included in the systematic review. Circular taping is minimally effective in relieving force action on the pulley system and is ineffective in preventing pulley injuries. The H-taping method leads to a significant decrease of the tendon–bone distance in the injured finger, which has positive effects for the prevention of tendonitis in the injured finger because of less friction over the edges of the remaining pulleys. Conclusion: We do not recommend using taping as a prophylactic method. We recommend using H-taping in the late phase of pulley injury rehabilitation. There is a need for additional randomized controlled trials.
