Načrtovanje dermalnih probiotičnih izdelkov za blaženje aken in atopijskega dermatitisa

Abstract
Koža je največji človeški organ, ki deluje kot fizična in imunska bariera, hkrati pa zagotavlja prebivališče stalni mikrobioti in preprečuje razrast patogenih mikroorganizmov. Neravnotežje v kožni mikrobioti prispeva k razvoju bolezni. Akne in atopijski dermatitis sta kronični kožni bolezni, ki sta močno povezani s spremembo v sestavi kožnih bakterij. Za akne je značilna prekomerna kolonizacija kože in foliklov s Propionibacterium acnes, za atopijski dermatitis pa kolonizacija kože s Staphylococcus aureus. V mikrobioti so poleg patogenih prisotni tudi koristni mikroorganizmi, imenovani probiotiki. Slednji so živi mikroorganizmi, ki imajo vrsto koristnih učinkov na kožo. Probiotični sevi se razlikujejo po probiotični aktivnosti in varnosti. Vgrajevanje probiotikov v kozmetične izdelke in dermatike je novo, hitro razvijajoče se področje. Izraz probiotik v teh izdelkih ne zajema le živih probiotičnih celic, ampak tudi njihove presnovke, lizate in mrtve bakterije. V magistrski nalogi smo zbrali podatke o vrstah probiotikov ter o študijah mehanizmov njihovega probiotičnega delovanja in vitro in ex vivo, ki bi lahko imeli ugoden vpliv na vzpostavitev normalne kožne mikrobiote in ublažili posledice njenega porušenega ravnovesja. Probiotične sestavine lahko na koži delujejo protimikrobno, imunomodulatorno, krepijo kožno bariero in/ali preprečujejo pritrjevanje patogenov. Iz objavljenih znanstvenih člankov in patentov smo pridobili podatke o različnih načinih formuliranja probiotičnih sestavin v dostavne sisteme, zbrali podatke o njihovem kliničnem preskušanju za blaženje aken in atopijskega dermatitisa ter na podlagi predpisov komentirali regulatorne omejitve pri uvajanju tovrstnih izdelkov na trg. Ugotovili smo, da raziskovalci odkritja iz predkliničnih študij dopolnjujejo z rezultati kliničnih študij in potrjujejo učinkovitost probiotičnih sestavin. Vsi dermalni probiotični izdelki, ki so trenutno dostopni na trgu v Evropski uniji in so namenjeni blaženju aken in atopijskega dermatitisa, ustrezajo predpisom za kozmetične izdelke. Ti vsebujejo dobro poznane probiotične seve oz. sestavine, pridobljene iz njih. Trenutno potekajo klinična preskušanja izbranih probiotikov kot zdravilnih učinkovin, kakor tudi postopki topikalne presaditve posameznih koristnih komenzalnih bakterijskih sevov. Z uporabo dermalnih probiotičnih izdelkov bi lahko podprli zdravljenje aken in atopijskega dermatitisa s konvencionalnimi učinkovinami tako, da bi okrepili kožno bariero, ali pa delovali protimikrobno in protivnetno, s čimer bi lahko zmanjšali lokalno ali sistemsko uporabo antibiotikov in/ali glukokortikoidov.

Title:Design of dermal probiotic products for mitigation of acne or atopic dermatitis
Abstract:
Skin is the body’s largest organ and is acting as a physical and immunological barrier. It also provides shelter for the resident commensal microbiota and prevents the pathogenic microorganisms to overgrow. An imbalance in skin microbiota encourages the development of skin diseases. Acne and atopic dermatitis are chronic skin diseases strongly connected to changes in the composition of skin bacteria. Typical for acne is an excessive colonisation of skin and follicles with Propionibacterium acnes, and for atopic dermatitis an excessive colonisation of skin with Staphylococcus aureus. Along with the pathogenic microorganisms, beneficial microorganisms called probiotics are present in the microbiota. They are live microorganisms that have a variety of beneficial effects on the skin. Probiotic strains vary in terms of their probiotic activity and safety. Integrating probiotics into cosmetic products and dermatics is a new and fast developing area. In these products, the term probiotic includes not only the live probiotic cells but also their metabolites, lysates, and dead bacteria. In this master thesis, we collected data on types of probiotics, in vitro and ex vivo studies of probiotic activity mechanisms that could have a beneficial influence in establishing a normal skin microbiota and ease the consequences of its imbalance. Probiotic ingredients can act antimicrobial and immunomodulatory, strengthen the skin barrier, or prevent pathogens from attaching. From scientific articles and patents, we gathered data on various ways of formulating probiotic ingredients into delivery systems, collected the data on clinical trials of the products designed to mitigate acne and atopic dermatitis, and commented the regulatory limitations on introducing the products in the market based on regulations. We determined that the results of preclinical studies are complemented by the discoveries from the clinical studies and confirm the effectiveness of probiotic ingredients. All dermal probiotic products designed to mitigate acne and atopic dermatitis that are currently available on the EU market comply with cosmetics regulation. In their formulation, these either include well-known probiotic strains, or ingredients sourced out of them. Currently, clinical trials of selected probiotics for active substances are in progress, as well as procedures of topically transplanting individual beneficial commensal bacterial strains. Dermal probiotic products could support the treatment of acne and atopic dermatitis with conventional drugs to help strengthen skin barrier or act antimicrobial and anti-inflammatory, which could diminish the local or systemic use of antibiotics and/or glucocorticoids.
