Abstract
Uvod: Pri pacientih z multiplo sklerozo in pacientih po možganski kapi je funkcija roke pogosto zmanjšana, kar vpliva na izvajanje vsakodnevnih dejavnosti. Za ocenjevanje fine motorike in spretnosti roke se pri obeh skupinah pogosto uporablja test devetih zatičev. Pri tem se meri čas, ki je potreben, da preiskovanec z eno roko zatiče posamično zatakne v luknjice in jih nato posamično iztakne in pospravi v posodico. Namen: Namen diplomskega dela je bil pregledati raziskave o merskih lastnostih testa devetih zatičev pri bolnikih z multiplo sklerozo in pri bolnikih po možganski kapi. Metode: Pregled literature je potekal v podatkovni zbirki PubMed in zajel raziskave o merskih lastnostih, ki so ustrezale merilom za vključitev. Zajete so bile raziskave, objavljene do konca januarja 2019. Rezultati: V pregled smo vključili enajst raziskav. Zanesljivost so preverjali v petih, veljavnost v sedmih in sposobnost zaznavanja sprememb v štirih raziskavah. Objavljene so bile od leta 2002 do 2017. Ugotovili so visoko do odlično zanesljivost posameznega preiskovalca pri bolnikih z multiplo sklerozo in po možganski kapi (ICC = 0,85–0,99) ter zmerno pri bolnikih po možganski kapi s spastičnostjo (ICC = 0,64). Odlično zanesljivost med preiskovalci so ugotovili pri bolnikih z multiplo sklerozo (ICC = 0,98). Ugotovili so boljšo sočasno veljavnost testa devetih zatičev pri ocenjevanju dejavnosti, kot telesnih funkcij in zmanjšano občutljivost pri bolnikih s spastičnostjo. Zaključek: Pregled raziskav je pokazal, da je test devetih zatičev veljavna in zanesljiva metoda za ocenjevanje fine motorike roke pri bolnikih z multiplo sklerozo ali pri bolnikih po možganski kapi. Potrebne so nadaljnje raziskave o zanesljivosti med preiskovalci pri obeh skupinah bolnikov. Smiselne bi bile nadaljnje raziskave o vplivu spastičnosti, ataksije, tremorja in kognitivnih motenj na merske lastnosti tega testa. Znane vrednosti najmanjše zaznavne spremembe lahko služijo kot temelj za raziskovanje in določanje vrednosti najmanjše klinično pomembne razlike pri obeh skupinah bolnikov.

Title:Measurement properties of nine hole peg test in patients with multiple sclerosis and after stroke – systematic literature review : diploma work
Abstract:
Introduction: In patients with multiple sclerosis and patients after stroke, the hand function is often impaired, which affects their daily activities. In both patient groups The Nine Hole Peg Test is often used to assess fine motor skills of the hand. It measures the amount of time needed to insert nine pegs one by one with one hand into a board and then remove them as quickly as possible. Purpose: The purpose of the thesis was to conduct a literature review on the measurement properties of Nine Hole Peg Test in patients with multiple sclerosis and patients after stroke. Methods: The literature review was conducted in the PubMed database. The included studies had to meet the inclusion criteria. The review was constructed at the end of January 2019. Results: Eleven studies were included in the review. Reliability was assessed in five, validity in seven, and ability to detect changes in four studies. They were published between 2002 and 2017. The results show high to excellent intra-rater reliability in patients with multiple sclerosis and patients after stroke (ICC = 0.85–0.99) and moderate intra-rater reliability in patients with spasticity (ICC = 0.64). The inter-rater reliability in patients with multiple sclerosis was excellent (ICC = 0.98). The concurrent validity of The Nine Hole Peg Test was greater in assessing activity than function. The sensitivity of the test was decreased in patients with spasticity. Conclusion: The literature review showed that The Nine Hole Peg Test is a valid and reliable method to assess fine motor skills of patients with multiple sclerosis and after stroke. Further research on inter-rater reliability is needed in both groups of patients. The literature review provides a basis for further research on the effects of spasticity, ataxia, tremor, and cognitive impairment on its measurement properties. The known values of the minimal detectable change can serve as the basis for the study of minimal clinical important difference in both patient populations.
