Abstract
Uvod: Za učinkovito rehabilitacijo otrok s cerebralno paralizo je potrebna ustrezna izbira terapevtskega programa, ki na otroka gleda celostno. Na področju rehabilitacije otrok s cerebralno paralizo sta razvojno-nevrološka obravnava in terapija senzorne integracije po Ayresovi med najpogosteje in najdlje uporabljenimi terapijami. Medtem ko se razvojno-nevrološka obravnava osredotoča na gibalno področje in hkrati upošteva omejitve s senzornega področja, se terapija senzorne integracije po Ayresovi posveča senzornim težavam in njihovim vplivom na razvoj motoričnih spretnosti. Kadar se terapiji uporabljata skupaj, uspešneje pripomoreta k izboljševanju otrokovega funkcioniranja. Namen: Z diplomskim delom smo želeli pregledati literaturo na temo učinkovitosti razvojno-nevrološke obravnave in terapije senzorne integracije po Ayresovi ter s pomočjo ankete pridobiti vpogled v uporabo obeh pristopov med delovnimi terapevti v Sloveniji. Metode dela: Uporabili smo mešano metodo. Podatke smo zbrali s pregledom literature, objavljene med leti 2001 in 2019, ter z anketami, ki smo jih poslali slovenskim delovnim terapevtov, ki izvajajo razvojno-nevrološko obravnavo, terapijo senzorne integracije po Ayresovi ali oboje pri otrocih s cerebralno paralizo. Rezultati: S pregledom literature smo ugotovili, da od 13 člankov, ki proučujejo učinkovitost razvojno-nevrološke obravnave pri otrocih s cerebralno paralizo, 6 potrjuje njeno učinkovitost. Od 10 člankov, ki proučujejo učinkovitost terapije senzorne integracije po Ayresovi, pa njeno učinkovitost potrjuje 7 člankov. Za učinkovit potek obravnave pri otrocih s cerebralno paralizo je treba upoštevati otrokovo diagnozo (82-% strinjanje), uporabljati enake, standardizirane ocenjevalne instrumente med obravnavo (100-% strinjanje), postavljati cilje skupaj s starši in otroki (100 % strinjanje), pomembno je kombiniranje elementov različnih metod (razvojno-nevrološka obravnava 100-%, terapija senzorne integracije po Ayresovi 94-% strinjanje) ter izvajanje domačega programa (razvojno-nevrološka obravnava 89-%, terapija senzorne integracije po Ayresovi 100-% strinjanje). Več kot polovica terapevtov, ki izvajajo razvojno-nevrološko obravnavo (54 %), ter več kot polovica tistih, ki izvajajo terapijo senzorne integracije po Ayresovi (56 %), sledi raziskavam na temo razvojno-nevrološke obravnave oziroma terapije senzorne integracije po Ayresovi pri otrocih s cerebralno paralizo. Razprava in zaključek: S pregledom literature ter vprašalnika smo ugotovili, da je učinkovitost rehabilitacije odvisna od zdravstvenega stanja otroka s cerebralno paralizo, obdobja začetka rehabilitacije, rednosti in intenzivnosti terapevtskih obravnav ter sodelovanja z otrokom in starši v času rehabilitacije. Rezultati so pokazali, da je za učinkovito obravnavo otrok s cerebralno paralizo treba izbrati ustrezno intervencijo glede na otrokovo diagnozo, med terapijo uporabljati enake, standardizirane ocenjevalne instrumente ter v okviru obravnave kombinirati različne metode.

Title:The application of neurodevelopmental treatment and sensory integration therapy in children with cerebral palsy : diploma work
Abstract:
Introduction: Efficient rehabilitation of children with cerebral palsy requires the selection of a suitable therapeutic programme, which treats the child holistically. In the field of rehabilitation of children with cerebral palsy, neurodevelopmental treatment and Ayres sensory integration therapy are among the therapies which have been used the longest and are among of the most frequently used. While neurodevelopmental treatment focuses on the field of movement, Ayres sensory integration therapy is dedicated to sensory difficulties and their impact on the development of motor skills. When the two therapies are applied simultaneously, they are more efficient in improving the child’s functioning. Purpose: The purpose of the thesis was to review the literature on the efficiency of neurodevelopmental treatment and Ayres sensory integration therapy and with the help of the survey gain insight into the use of both approaches among occupational therapists in Slovenia. Methodology: We used the mixed method. Data was collected by reviewing the literature published between 2001 and 2019 and by surveys which were sent to Slovenian occupational therapists who use neurodevelopmental treatment, Ayres sensory integration therapy or both in children with cerebral palsy. Results: 5 out of 10 articles which studied the efficiency of neurodevelopmental treatment and 7 out of 10 articles which studied the efficiency of Ayres sensory integration therapy confirmed their effectiveness in children with cerebral palsy. The efficient treatment of children with cerebral palsy requires taking into consideration the child's diagnosis (82% agreement), use of identical, standardized assessment instruments in the course of the treatment (100% agreement), setting the goals in cooperation with the parents and children (100% agreement), combining elements from various methods is important (neurodevelopmental treatment 89%, Ayres sensory integration therapy 100% agreement), as is carrying out the home programme (neurodevelopmental treatment 89%, Ayres sensory integration therapy 100% agreement). More than one half of the therapists who use neurodevelopmental treatment and those who use Ayres sensory integration therapy (54% and 56%, respectively) follow research on neurodevelopmental treatment or Ayres sensory integration therapy in children with cerebral palsy. Discussion and conclusion: By reviewing the literature and the questionnaire, we established that the rehabilitation efficiency depends on the health condition of the child with cerebral palsy, the period of the beginning of rehabilitation, the regularity and intensity of the therapeutic sessions and the cooperation with the child and the parents in the course of the rehabilitation. The results indicate that the efficient treatment of children with cerebral palsy requires the selection of a suitable intervention in accordance with the child’s diagnosis, the use of identical, standardized assessment instruments during the therapy and the combination of various methods in the framework of the treatment.
