Abstract
Uvod: Ena od ključnih dilem v zdravstveni oskrbi je vprašanje, kako dolgo vztrajati pri aktivnem zdravljenju, ko je upanje na izboljšanje stanja slabo in lahko pričakujemo le še nadaljnje poslabševanje. Pacient lahko izrazi svojo voljo z vodilom »ne oživljaj«, vendar pa se tako pri uvajanju kot upoštevanju tega vodila v praksi pojavljajo etične dileme. Namen: Namen diplomskega dela je z etičnega vidika raziskati uporabo vodila »ne oživljaj« in različna stališča glede upoštevanja tega vodila v zdravstveni negi. Metode dela: V diplomskem delu smo opravili pregled literature in uporabili 22 znanstvenih člankov. Rezultati: Sistematični pregled literature kaže, da se etične dileme pri uvajanju in upoštevanju vodila »ne oživljaj« postopoma razrešujejo. Še vedno pa se pojavljajo težave zaradi posploševanja uporabe vodila ter vpliva tega na izvedbo posegov, potrebnih pri zdravljenju pacientov. V literaturi je opisano veliko dilem, ki bi jih bilo mogoče rešiti z boljšo komunikacijo med pacienti in zdravstvenim osebjem, prek katere bi obe strani pridobili pomembne informacije za sprejemanje odločitev ob koncu življenja. Pomembno vlogo pri spodbujanju in vzdrževanju komunikacije ima medicinska sestra, ki med drugim skrbi za pridobivanje informacij, vodenje pacientove dokumentacije in upoštevanje vodil, ki so zapisana v pacientovi dokumentaciji. Razprava in sklep: Funkcija medicinske sestre pri uvajanju vodila »ne oživljaj« je pomembna z vidika komuniciranja in informiranja med pacientom in zdravstvenim timom. Jasne smernice in ustrezna pravna ureditev vloge medicinske sestre v zdravstvenih ustanovah bi imela pomemben učinek na zmanjšanje dilem in napak v zvezi z vodilom »ne oživljaj«.

Title:Respecting the patient's will "do not resuscitate" in nursing care - literature review : diploma work
Abstract:
Introduction: One of the most important unresolved issues in health care is the question of continuation of care when prognosis is poor and further deterioration is inevitable. In this respect, the patient can express his or her will with the "do not resuscitate" order, but in practice, ethical issues arise with the introduction and consideration of such directives. Purpose: The purpose of this thesis is to research the use of the "do not resuscitate" order and the various points regarding its use in health care from ethical point of view. Methods: Review of 22 relevant scientific articles was performed. Results: Systematic review of the literature shows ethical issues regarding introduction and implementation of the "do not resuscitate" order are gradually being resolved. However issues often reappear with regard to the generalization of the order and its influence on treatment decisions. Many ethical issues can be resolved with better communication between patients and healthcare providers, through which both sides gain important information in end-of-life decision making. Besides nurse's key role in promoting and maintaning communication, nurse plays an important role in obtaining information, keeping patient's records and taking into account the »do not resuscitate« order in the patient's documentation. Discussion and conclusion: Nurse's function in the introduction and implementation of the "do not resuscitate" order is important with respect to communication between patient and health care providers. Clear guidelines and appropriate regulations of the nurse's role in health care institutions would have a significant impact on resolving the issues and on the reduction of errors in relation to the "do not rescucitate" order.
