Abstract
Radioterapija je metoda zdravljenja raka z ionizirajočim sevanjem. Pri tem je bistvenega pomena zanesljivost sistema, ki med obsevanjem zagotavlja enakost lege pacienta tisti legi, v kateri je bil pripravljen obsevalni načrt. Sistem ABC (Active Breathing Coordinator™) je eno od orodij, ki zagotavljajo ponovljivost obsevanja v določeni dihalni fazi, kar smo z meritvami tudi potrdili. Sistem ABC smo pripravljali za namen obsevanja v globokem vdihu. Zaključna naloga opisuje testiranje sistema ABC na 16 prostovoljcih. Pomerjena je bila prostornina zajetega zraka med prostim dihanjem in prostornina zraka zajeta pri globokem vdihu, ter lega prsnega koša. Izračunali smo ponovljivost lege prsnega koša ob konstantnih nastavitvah sistema ter udobnost teh nastavitev za posameznega prostovoljca. Ugotovili smo, da je optimalna nastavitev praga prostornine zadržanega globokega vdiha odvisna ne le od vrednosti volumna maksimalnega globokega vdiha, pač pa tudi od prostornine vdihnjenega zraka med prostim dihanjem. Glede na izsledke je predlagan prilagojen algoritem izračuna optimalne nastavitve praga prostornine zadržanega globokega vdiha pri pacientih, s katerim bi lahko omogočili obsevanje v fazi zadržanega globokega vdiha tudi populaciji bolnikov, ki jih sicer ne bi mogli obsevati na ta način. Med prostovoljci so se tovrstne težave pojavile pri 20 % prostovoljcev.

Title:Calibration and optimization of the ABC (Active Breathing Coordinator ™) system for respiratory phase monitoring during radiotherapy
Abstract:
Radiotherapy is a method for treating cancer with ionized radiation. It is of crucial importance to reposition the body in the same position in which the radiation plan was constructed. The ABC system (Active Breathing Coordinator™) is a tool that allows us to treat the patient only while in a particular phase of his breathing cycle. The performed tests were limited to the deep-inhale-breath-hold (DIBH) mode. This thesis describes the testing of the ABC system on 16 volunteers. The measurements recorded for each volunteer were the tidal volume, the maximum volume of an inhalation, the chest position and the comfort level during DIBH phase. The conclusion was that the optimal DIBH threshold is not only related to the lung capacity, but also to the tidal volume of the participant. With these findings an alternative algorithm was suggested to determine the settings of the optimal DIBH volume of inhaled air during the treatment, with which we could expand the range of patients that can be treated using this method – in the presented study this new algorithm increased the number of patients able to be treated with the ABC system by 20 %.
