Abstract
Uvod: Kronična nestabilnost skočnega sklepa je posledica več zaporednih zvinov skočnega sklepa in se kaže kot mehanska in/ali funkcionalna nestabilnost. Simptomi, ki so značilni za kronično nestabilnost se pokažejo pri kar 70 % aktivne populacije, ki je vsaj enkrat utrpela zvin. Poškodbe sklepa vplivajo na sklepno gibljivost, občutek za zaznavanje položaja in gibanja sklepa, povzročajo bolečino in zmanjšano funkcijo, ob nepravilni ali odsotni intervenciji pa lahko povzročijo degenerativne spremembe sklepa. Namen: Preko pregleda literature predstaviti učinkovitost različnih manualnih tehnik sklepne mobilizacije pri konservativnem zdravljenju pacientov s kronično nestabilnostjo skočnega sklepa. Metode dela: Literatura je bila pregledana v elektronskih podatkovnih zbirkah MEDLINE, MEDLINE in Process, Embase, AMED, PsycINFO, CLINAHL, PEDro, PUBmed, ScienceDirect, Elsevier, Sportdiscus in Cochrane Library ter izbrana po opisanih vključitvenih in izključitvenih kriterijih. Končno je bilo v analizo vključenih šest člankov. Rezultati: V vključenih raziskavah so primerjali učinke sklepne mobilizacije po Maitlandovem in Muliiganovem konceptu s skupino brez intervencije, ali pa so primerjali sklepno mobilizacijo z ali brez obremenitve. V raziskavah so sodelovali preiskovanci, z ponavljajočimi zvini in kronično nestabilnostjo skočnega sklepa. Učinke sklepne mobilizacije so ocenjevali z meritvami sklepne gibljivosti, dinamičnega in statičnega ravnotežja, bolečine in samo-opisane nestabilnosti. Razprava in zaključek: Pregled analiziranih člankov je pokazal, da manualne tehnike po Maitlandovem in Mulliganovem konceptu vplivajo na takojšnje, kratkoročno in dolgoročno izboljšanje pasivnega in aktivnega obsega giba dorzalne fleksije, izboljšanje statičnega ravnotežja in zmanjšanje pogostosti samo-opisane nestabilnosti. Izboljšanja ne moremo potrditi pri dinamičnem ravnotežju, bolečini in vzdražnosti avtonomnega in somatskega živčevja. Po pregledanih člankih se je sklepna mobilizacija z obremenitvijo po Mulliganovem konceptu izkazala za najbolj učinkovit pristop.

Title:Effect of joint mobilisation in patients with chronic ankle instability - literature review : diploma work
Abstract:
Introduction: Recurrent sprains can cause chronic ankle instability that lead in to mechanical and/or functional instability. Symptoms typical for chronic ankle instability occur in 70% of the active population, who had their ankle sprained at least once. Injury affects range of motion, motion and position sence of the joint, causes pain and dysfunction, and if not treated correctly joint degenerating. Purpose: To overview and compare the effectiveness of different joint mobilization techniques when threating patients with chronic ankle instability. Methods: Search for studies was performed in databases MEDLINE, MEDLINE in Process, Embase, AMED, PsycINFO, CLINAHL, PEDro, PUBmed, ScienceDirect, Elsevier, Sportdiscus and Cochrane Library. Finally, six studies were included which met the inclusion and exclusion criteria. Results: Overviewed studies compared effectiveness of joint mobilization techniques described by Maitland and Mulligan with no intervention, some also compared weight bearing and non-weight beating joint mobilization techniques. Participants included in the studies all suffered from chronic ankle instability or recurrent sprains. Effectiveness of joint mobilization was measured with changes in range of motion, dynamic and static balance, pain and self-reported instability. Discussion and conclusion: Literature overview shows that manual techniques of joint mobilization as described by Maitland and Mulligan have immediate, short term and long term benefits on passive and active range of motion of dorsiflexion, static balance and self-reported instability. It is not possible to confirm benefits on dynamic balance, pain, autonomous and somatic nervous system. Overview shows that Mulligan weight bearing joint mobilization gives best results.
