Abstract
Avtoimunske bolezni so heterogena skupina bolezni, katerim so skupni porušeni mehanizmi za ohranjanje neodzivnosti do telesu lastnega. Terapija CAR T spada med napredne celične terapije, kjer pacientu največkrat odvzamejo njegove lastne celice T, jih namnožijo in vrnejo nazaj v telo. Vendar pa se da celice tudi genetsko preoblikovati, kar je temelj terapije CAR T. V odvzete celice T se vnese nov genetski material, ki jim omogoča, da na svoji površini izražajo himerni antigenski receptor. CAR je namreč fuzijski protein, himerni antigenski receptor, ki je konstruiran glede na to, katera komponenta se bo vezala na ta receptor, kar pa je povsem odvisno od patologije vsake bolezni. CAR je v osnovi sestavljen iz zunajcelične domene, ki se veže na antigen, zunajceličnega vmesnika, transmembranske domene in znotrajcelične signalne domene. Zaenkrat so znani trije načini zdravljenja avtoimunskih bolezni s terapijo CAR T. Prvi deluje tako, da so celice T z izraženim CAR usmerjene proti avtoreaktivnim celicam T, pri drugi metodi so tarča himernega receptorja avtoreaktivne celice B, pri tretji pa so uporabljene regulatorne celice T, ki se prav tako borijo proti avtoreaktivnim celicam T.

Title:Chimeric antigen receptor (CAR) T cell therapy for treatment of autoimmune diseases
Abstract:
Autoimmune diseases are a heterogeneous group of diseases, that results from a dysfunction of the immune system, where an abnormal immune response is triggered to a normal body part. CAR T therapy is of the novel adoptive cell therapies, where the patient has his own T cells removed, proliferated and put back into the body. The cells can also be genetically engineered, which is the foundation of the CAR T therapy. New genetic material gets inserted into the isolated T cells, that enables them to express the chimeric antigen receptor on their surface. CAR is a fusion protein, a chimeric antigen receptor, that is designed according to target binding onto the receptor, which is dependent on the pathology of the disease. CAR is composed from extracellular domain, that binds to the antigen, extracellular spacer, transmembrane domain and intracellular signalling domain. So far, there are three treatment methods for autoimmune diseases with CAR T therapy. First method uses T cells with CARs targeted against autoreactive cells, while chimeric receptor in the second method targets autoreactive B cells. Regulatory T cells are used to fight against autoreactive cells T in the third method.
