Abstract
Uvod: Obstipacijo štejemo med motnje odvajanja, ki lahko prizadene vsakega bolnega posameznika. Obstipacija ni bolezen, ampak zelo pogost akutni in kronični zdravstveni problem, ki nastane zaradi številnih vzrokov. Pooperativna obstipacija pri ortopedskih pacientih nastane predvsem pri starejši populaciji zaradi uporabe opiatnih analgetikov, komorbidnosti, omejenega gibanja, hranjenja in pitja tekočin. Posledično pacienti trpijo zaradi stresa, zapletov, povezanih z obstipacijo, podaljšuje se hospitalizacija in izrazito se poslabša kakovost življenja. Namen: Namen diplomskega dela je na teoretični ravni opredeliti obstipacijo po ortopedski operaciji, možnosti njenega preprečevanja, zdravljenja in obvladovanja z nefarmakološkimi pristopi z vidika zdravstvene nege. Metode dela: Uporabljena je bila deskriptivna metoda dela. Opravljen je bil pregled strokovne in znanstvene literature v angleškem in slovenskem jeziku z določenimi omejitvami: zadetki s polnim besedilom, znanstvene in strokovne recenzirane revije, časovno obdobje med letoma 2000 in 2017. Razprava in zaključek: Zdravstvena nega ima pri obvladovanju pooperativne obstipacije edinstveno in zelo pomembno vlogo. Medicinska sestra mora poznati z dokazi podprte nefarmakološke pristope in smernice za preprečevanje in zdravljenje obstipacije. Preprosti in poceni nefarmakološki pristopi, kot so: promocija gibanja, ustrezni prehranski režim, hidracija pacienta, abdominalna masaža, zavzemanje optimalnih položajev telesa pri odvajanju, metoda povratne biološke zanke, zagotavljanje zasebnosti in detabuizacija odvajanja, so dolgoročno učinkoviti in pacientu zagotavljajo boljšo kakovost življenja. Zdravstvena nega potrebuje nadaljnje raziskave v smeri iskanja novih dokazov za nefarmakološko obvladovanje obstipacije. Implementirati je treba obstoječe nefarmakološke pristope v prakso in ne nazadnje okrepiti interdisciplinarno sodelovanje.

Title:Nonpharmacologic approaches to management of obstipation after orthopedic surgery : diploma work
Abstract:
Introduction: Obstipation is one of the defecation disorders, that can affect every patient. Obstipation is not a disease, but common acute or chronical issue, which can be caused by many reasons. Post operative obstipation with orthopedic patients is mostly present within elderly population, due to use of opiod analgetics, comorbidity, reduced mobility and nourishment. Consequently, patients are under stress, suffer from obstipation side effects, and therefore hospitalisation can be prolonged and it affects their quality of life. Purpose: The purpose of this work is to theoretically resolve obstipation post orthopedic surgery, prevention possibilities, treatment and nursing management with non pharmacological approach. Methods: Descriptive method has been used with review of scientific literature in English and Slovenian language with the following limitations: full-text articles, scientific and reviewed journals, time period between 2000 and 2017. Discussion and conclusion: Nursing has an important role in managing postoperative obstipation. Nurse has to be familiar with non pharmacological approaches and guidelines for prevention and treating obstipation. Simple and not expensive methods as mobility promotion, convenient nourishment, hydration, abdominal massage, appropriate body position during defecation, biofeedback method, ensuring privacy and demolishing taboos regarding defecation, have long term effect and can provide patients with better quality of life. Nursing, however, requires additional research in direction of finding new proves for non pharmacological obstipation management. It is of great importance to implement existing methods in clinical practice and aditionally strengthen interdisciplinary cooperation.
